Thyroid Cancer that has metasis to the Lungs

My very close friend was just diagnosed with Stage 4 thyroid Cancer and we are completely lost with treatment options , if there are any at this point. If anyone can give us some experience. It has spread to the Lungs and lymph nodes . He is feeling a lot of pain , does this mean it is really advanced?
