Generally speaking a 3 cm mass in the bladder is usually bladder cancer (transitional cell carcinoma). Given your description it sounds like a superficial tumor (not invading the muscle of the bladder) which is a good thing. Superficial cancers are the most common and the most common cause is smoking so if you currently smoke quit. Squamous cell cancer can occur but it is very uncommon. Less than 5 cm is considered a small tumor. You can read more about bladder cancer on this website, good luck
